Physician Referrals

Protect Health offers electronic patient referrals! Simply fill out the Physician Referral Form by clicking the button below and the information will be uploaded into our HIPPA-secure EHR. Once I receive notification of your referral, I will reach out directly to your patient within 3-5 business days to help them learn more about working with me. We are in network with Horizon Blue Cross Blue Shield, Aetna and Cigna however, insurance coverage varies.
